Four Posts

Why a whole jaw can rest on four implants.

At the back of the mouth, bone often thins after years without teeth or under a denture. So the two rear posts are tilted forward toward firmer bone near the front, four posts can hold the entire arch, and the back of the jaw often needs no rebuilding first.

After: the finished arch

An arch only loads evenly if every post sits where it was planned, so the angle, depth and position of each one are fixed on the cone-beam scan and carried into the mouth by a printed guide.All-on-4 explained in full

The Sequence

Scan, implant day, healing, then zirconia.

Scan and decide

An examination and a cone-beam scan with Dr. Nahlah show whether your bone can take teeth on day one. Cost, insurance and financing are settled at this visit too.

Implant day

Any teeth beyond saving are removed that day if your case allows it, the four posts are placed under local anesthetic plus nitrous oxide or oral sedation, and a fixed PMMA arch is screwed on before you head home.

Three to four months

The posts fuse while you keep to soft foods. Reviews along the way check on the bone and adjust the provisional teeth as the gum settles.

The final arch

The zirconia arch, cut on the milling machine in the building, replaces the provisional. Healing around it takes a further two to four months; after that it gets the same brushing and flossing as your own teeth.

Arch Types

Teeth only, or teeth and gum: ask which you are being quoted.

Two offices can both call their work All-on-4 and fit very different arches. What separates them is the share of tooth against the share of pink acrylic that replaces missing gum and bone. Here the plan starts from the slimmest arch your bone will carry.

FP1

Teeth alone

  • Reads as natural crowns and bridges
  • No pink acrylic and no added bulk
  • Usually where the gum line is still natural
FP2

Longer teeth

  • Teeth made longer, with a little pink
  • Any pink follows the line of the gum
  • Seldom the best choice
FP3

Teeth, gum and bone

  • A thick pink base replaces lost gum and bone
  • The thicker and stronger prosthesis
  • Often right where bone loss is advanced, and simpler to keep clean
FP1, FP2 and FP3 full-arch prostheses side by side
One arch, built three ways; the pink is what changes.

Where bone loss has gone far, FP3 is the honest plan and also the sturdier one. Whoever quotes you, ask which of the three they intend, and why.Raise it at your consultation

Questions

Full arches for Great Falls patients: questions.

Do I have to go without teeth at any point?
For most All-on-4 patients, no. Failing teeth are usually removed on the day the posts go in, when the case permits, and the fixed provisional arch is fitted at that same visit, so you drive home to Great Falls with teeth, or are driven if you had oral sedation.
How many trips to Tysons Corner does a full arch take?
The plan runs in four stages: the consultation and scan, implant day, reviews during the three to four months of healing, and the day the zirconia arch is fitted. Each review checks how the bone is taking and adjusts the provisional teeth.
Can I eat normally with the provisional arch?
It is a working set of teeth in PMMA, fixed in place, but it is a stage rather than the result. Keep to soft foods while the posts fuse; once the zirconia arch is in, it is looked after like your own teeth.
What if a denture has already worn my jawbone down?
That is often why All-on-4 is the choice: its angled rear posts find denser bone further forward. Some mouths still need a graft first, and your scan will say whether yours does. How All-on-4 handles bone loss →
Could my upper and lower teeth be replaced together?
Yes. Both jaws can be designed as a single case, so upper and lower meet evenly; the gallery calls these double-arch cases. Both arches on implants →
What care does a fixed arch need once it is finished?
Brush and floss it as you would natural teeth; it stays in at night. Book a cleaning every six months: the one condition the warranty sets. The five-year warranty →
