Implant procedures
The implant work this practice carries out is covered for five years. The post is designed as a lifetime replacement root; the parts above it are what a warranty is for.

That is the whole of it, and it is the sentence the practice has published since before this site was built. Replacement is free of charge: no deductible, no pro-rata scale that pays less in year four than in year one, and no charge for the appointment at which the replacement is fitted.
The implant work this practice carries out is covered for five years. The post is designed as a lifetime replacement root; the parts above it are what a warranty is for.
Veneers, crowns and the rest of the cosmetic work designed and fitted here are covered on the same five-year terms.
A full arch carries the warranty too, on the same condition as everything else: the six-monthly Precare cleanings.

The warranty is conditional on Precare, the practice's own maintenance program: general cleanings every six months. That is the same interval any set of teeth needs, and it is the only condition attached.
The reason is not administrative. What ends most implant work early is not the implant, it is the gum and bone around it, and six-monthly cleaning is what catches that while it is still reversible. A practice that guarantees its work for five years needs to see the work during those five years.
There is no form and no claims department. Ring the office, describe what has happened, and you will be seen. Second opinions on implant surgery are free, whether the original work was done here or somewhere else.
