Same-Day Dental Implants: Who Qualifies

From the practice of Dr. Esam Nahlah, DDS, MS, board-certified prosthodontist

27 October 2026

You qualify for same-day dental implants when the implants can be placed steady enough to carry a set of provisional teeth on the day they go in. An examination and a cone-beam scan at the consultation show whether that is likely for you, and the implants themselves have to be stable once placed. For a full arch on All-on-4, the answer at this practice is yes in most cases, and bone loss often counts for you rather than against you.

The treatment itself, step by step, is on the same-day dental implants page. This article is about candidacy: what has to be true for teeth on the day, and what happens if it is not.

What "same day" actually means

Same-day teeth are real teeth, for now. On implant day you receive a fixed provisional arch made of PMMA, a durable acrylic. It is screwed down to the implants, not a removable denture, and it is fully functional.

It is a stage, not the result. For three to four months you wear it while bone grows onto the implants, eating soft foods, with reviews along the way to check integration and adjust the provisional. Then the final arch, in zirconia, replaces it, and two to four more months finish the healing around it. From then on it is brushed and flossed like natural teeth.

So for most All-on-4 patients, "same day" means going home with fixed teeth rather than none. It does not mean the treatment is finished in a day.

Bone, health and bite requirements

Bone comes first. The implants have to be stable in the jaw on the day they are placed; if they are not, the teeth have to wait. The cone-beam scan measures the bone you actually have before anything is decided. With All-on-4, the two rear implants are angled forward into the denser bone at the front of the jaw, which is why bone loss at the back of the mouth often does not rule you out. Where bone is too thin, grafting may be needed first.

Health matters because it affects how bone heals around an implant:

  • Gum disease is treated first, so each implant has healthy gum around it.
  • Smoking, diabetes and the medicines you take all affect healing. Tell the team about them, and the plan takes them into account.
  • Your medical history and medications are gone through at the consultation.

The bite matters because a full arch has to carry chewing forces evenly. Each implant has to land at an exact angle, depth and position, or the arch above it will never load evenly, which is why placement is planned on the scan and done through a printed surgical guide. If you clench or grind your teeth, say so at the consultation, since heavy forces matter while implants heal. And the soft-food diet during those months is not optional.

Single tooth vs. full arch

At this practice, All-on-4 is the treatment behind same-day teeth. Four implants carry a whole upper or lower arch, and in most cases the provisional is fitted before you go home. It suits people whose remaining teeth are failing or already gone, and denture wearers tired of a plate that rubs and slips. Where both arches are being replaced, the same applies to each.

A single tooth can sometimes be replaced the same day. Where the case allows, the implant goes in at the time of the extraction, and qualifying patients leave with a temporary tooth. The scan shows whether yours qualifies. If it does not, a single dental implant integrates for about four months, with a temporary worn throughout, before the final tooth goes on.

What happens if you don't qualify on the day

Not qualifying for teeth on the day is not the same as not qualifying for implants. Many patients qualify after bone grafting or extractions. What changes is the order of treatment:

  • If bone is too thin, it is grafted first. Grafting is its own procedure with several months of healing, and the teeth wait for the bone to heal.
  • If gum disease is present, it is treated before any implant goes in.
  • If the implants are not steady enough to carry teeth straight away, they are left to integrate first, and in many cases you wear a temporary in the meantime.

Grafting, where it is needed, is written into your timeline at the first consultation, and you leave that visit knowing whether you qualify, what it costs and how long it takes. Implant treatment as a whole runs from a few months to over a year, depending on whether bone has to be rebuilt first.

If another dentist has called your case too complex, a second opinion here costs nothing. Bring the plan and the scan.

Questions

Does bone loss rule out same-day implants?

Often not. With All-on-4, the two rear implants are angled toward the denser bone at the front of the jaw, so bone loss at the back is often the reason All-on-4 is chosen rather than a reason against it. Some cases still need grafting first, and then the teeth wait for the bone to heal.

Can I be sedated for same-day implant surgery?

Yes. Implant day is done under local anesthetic with nitrous oxide or oral sedation, given at the office by the practice's own team, so you stay relaxed throughout. The practice does not offer IV sedation. After oral sedation, someone needs to drive you to and from the appointment.

Whether same-day teeth are likely for you can only be judged from an examination and a scan of your own jaw. A consultation includes the cone-beam scan and a straight answer on candidacy, cost and timeline, before you commit to anything. Book a consultation at the McLean or Alexandria office.
